By accepting this release and waiver form I am warranting that The New Era Tennis Academy has my permission to use my and/or my child(ren)’s and members of family’s images in photographs and/or videos taken throughout the tennis lessons, practices, tournaments and/or summer camps on and around the tennis courts which the Academy will occasionally use on their website (http://www.neweratennisacademy.com) and/or their social media sites such as Facebook or Instagram, flyers and banners.
